Sheila Ishii’s Terms and Conditions
Each clinician working at Metro Maryland Counseling Center, LLC has their own Terms and Conditions for their respective practice. Sheila Ishii’s terms cover: Process of Treatment: Confidentiality Ethics; Insurance, Fees, and Payment for Services Agreement; Confidentiality Specifics; Cancellations Policy; Emergency Contact Protocol; Use of Phone Calls, Emails, Texts; Legal Policy; Termination of the Treatment Relationship; How I Conduct Therapy; HIPPA requirements; Informed Consent with signature. These pages are discussed with the client in the initial diagnostic interview. The client has a chance to ask questions and receives a copy of the signed paperwork in the second session. Specialized consents are discussed when they become necessary. Telehealth Services is an example of this.
Emily Scott’s Terms and Conditions
Emily Scott Professional Counseling, LLC issues Terms and Conditions paperwork prior to a client’s intake session, for review and signature during said session. These forms include, but are not limited to, HIPAA: Client Rights and Therapist Duties (Limits of Confidentiality, Complaints, Use and Disclosure of Private Health Information); Therapy Consent, Policies & Agreements (Therapeutic Process, Confidentiality, Insurance, Fees, & Payment, Emergency Contact, and Consent), and Good Faith Estimate; clients are required to review and sign these upon intake. For minor clients, the signature of a parent or guardian is required for the minor to enter into a treatment contract. Forms may be updated or changed at the therapist’s discretion and will be reviewed with clients if needed. Clients are provided the opportunity to ask questions about the above Terms and Conditions prior to signing. SMS information is not shared with third parties without the client’s written authorization and it is never used for marketing purposes.
